Enrollment Checklist

• Copy of current vaccination record, must include bordatella ( kennel cough ). This is for the cleanest and safest environment for your dog and dogs already enrolled in the program.

• Medications and instructions for administering them.
  Prescriptions, heartworm medication, flea and tick preventative, etc.

• Any special needs, allergies etc.

• We feed and highly recommend Eukanuba Sporting brand dogfood.

• Females: Date of last heat cycle.

• Copy of 3 generation pedigree. ( optional )
  I like to know the background of dogs in training. Sometimes a particular strength or weakness arises in training and looking at the pedigree will help shed some light on these situations.
  I also have a number of people approach me about certain dogs in the program and having a background on dogs is really helpful.

• Hunt Test dogs:
  AKC registration number ( AKC Hunt Tests )
  UKC registration number ( UKC / HRC Hunt Tests )
  Dogs to be handled in hunt tests must be registered with parent registries.
  - HRC Hunt Tests must be registered with UKC.
  - AKC Hunt Tests must be registered with AKC.

• Payment for 1st month of training due at dropoff
